The Care Advantage program is designed to give our patients, residents, and their families a way to communicate with us regarding individuals or facilities who have performed above expectations in providing quality care. Care Advantage is also designed to allow our patients, residents and their families to express any concerns they may have about the operations in our facilities or services provided. If you have an area in which you feel that this facility or any employee is not meeting your needs as a patient or resident, or the needs of a loved one, please call the Care Advantage Customer Response Line with your comments. We will do our very best to respond to your concerns.
Directions: Off of 1-95 take exit 80 East, Route 406 (Garden Street). Continue on Garden Street to U.S. 1 and turn left. Follow U.S. I North to Jess Parrish Court, approximately 1.5 miles. Turn right on Jess Parrish Court. We are located 1/4 mile on the right.

SKILLED NURSING SERVICES
Skilled nursing services are suitable for patients who may have suffered from a prolonged acute or chronic illness. Skilled nursing services may also be required by patients who may have been injured or recently undergone surgery.
REHABILITATION SERVICES
Rehabilitation patients may require a wide-range of services, depending on their individual condition and diagnosis. Rehabilitation services include:

CASE MANAGEMENT
Our case management system provides sound recommendations regarding the plan of care for each patient/resident. During care delivery, each plan is re-evaluated by all members of the team and the levels of care are adjusted according to patient/resident progress and needs. This care planning process is an integral part of our team's ability to ensure that goals are met as quickly as possible.

GENERAL NURSING & RESTORATIVE CARE
We provide general and restorative care to residents who cannot be taken care of at home due to the caregiving demands of a chronic illness, the frailties of age, or physical limitations. Our focus is on providing care and comfort while striving to enhance the joy and dignity of life. We also offer an array of social events and recreational activities. Our general and restorative services include:

DIETARY SERVICES
Residents are served three well-balanced, nutritious meals with many menu options and the availability of snacks. We have registered dietitians available to accommodate any special diet needs and provide physician-ordered therapeutic diets.
RESPITE CARE
Our respite care program is a short-term temporary solution for caregivers who need a break. Our program includes nursing care, medication admini- stration, three meals a day, assistance with meals, grooming, dressing, as well as recreation and social programs.
